- Download to print, sign and return the Hui Nalu Registration Form to begin your Membership Profile for 2023. Provide a copy of a photo ID that includes a Date of Birth (passport, birth certificate, drivers license…) Provide a digital headshot photo, no hats or sunglasses please. If submitting via email, make sure it’s a jpg.
